Medical Questionnaire

There are two questionnaires to choose from . The choice depends on your answer to this question:
"Have you sent me your questionnaire and received advice from me before?"
NO - therefore a Full Medical Questionnaire is needed
- If your answer is NO, then you need to complete the Full medical questionnaire. Use this if you want to fill in the questionnaire on your computer[1]. Don't forget to copy / paste in details of the test(s) you require at the bottom of the form before you save it on your PC. Return to me by attachment to an email or by post.
- You can also request a "by hand" version of the questionnaire if you do not want to download it onto your computer. Email Hania or telephone the office on 01547 550 331.
YES - therefore a Short Medical Questionnaire is needed
- If your answer is YES, then you do not need to fill the full questionnaire. However, I will still need some information from you. Please, complete the short medical questionnaire. You can download it here: Short medical questionnaire [1] or ask for a paper copy to be sent to you.
- The completed questionnaire and your payment will constitute your order.
